Who We Serve

The people we serve come to Partners Creating Community (PCC) as individuals – each with unique experiences, needs, goals, and dreams. As such, our programming is customized for each individual. This may include living support, job coaching, social skills training, life skills development, recreational activities, social events, community integration, and more. We also provide resources, referrals, and crisis intervention to ensure those we serve receive the support they need.

Neurodivergence

The term “neurodivergence” refers to the broad range of variations within the brain that affect how people think, process information, and interact with the world. PCC serves neurodivergent adults with varying strengths and abilities who are on the autism spectrum. 

Intellectual Disability

Intellectual disability is characterized by limitations in abilities like reasoning, problem-solving, planning, abstract thinking, judgment, academic learning, and learning from experience. It can also impact practical skills needed to live independently and function in everyday life - like communication, social interactions, and self-care. PCC serves adults who are living with an intellectual disability. 

Mental Health Challenges

Mental health challenges encompass a broad range of emotional, psychological, and social well-being issues that can affect thoughts, feelings, behaviors, and overall functioning. These challenges influence how individuals cope with stress, make decisions, build relationships, and manage daily life. PCC proudly provides support for adults facing various mental health challenges, including Schizophrenia Spectrum Disorders, which affect thought and perception; Bipolar Disorders; Depressive Disorders; Anxiety Disorders; Obsessive-Compulsive Disorders; Trauma-Related Disorders; and others. Recognizing the diverse nature of these conditions, we offer compassionate care and personalized programs to help individuals build resilience, create meaningful relationships, foster independence, and achieve their personal goals.

The beauty of PCC lies within the diversity of the human condition you’ll find throughout our organization. Many even come to PCC with co-occurring conditions.
